The MMH+ application has been replaced by Patient360 as Anthem's longitudinal patient record, accessible via Availity. To access Patient360, please visit Availity using Google Chrome (www.availity.com). Once there, select the 'REGISTER' link and follow the Availity registration process. If you have any questions or concerns, please contact your Anthem Provider Representative. Anthem employees, please contact the IT service desk and submit a request for access to M360.
